PURSUANT TO 54 Pa.C.S.A. SECTION 701 ET SEQ.
AND NOW, comes Lavon Cheryl DuCharme, by and through O'Brien, Baric and
Scherer, Attorneys at Law and makes the following Petition:
1. Petitioner, Lavon Cheryl DuCharme, is an adult individual residing at 6185
Run Cross Lane, Enola, Cumberland County, Pennsylvania 17025.
2. Petitioner was born March 30, 1963 in Allegheny County, Pennsylvania.
3. At the time of birth, Petitioner was given the name Lavon Cheryl Thomas.
A copy of the birth certificate evidencing this fact is attached hereto and incorporated
herein as "Exhibit A."
4. On September 13, 1986, Petitioner married Michael J. DuCharme in Penn
Hills, Allegheny County, Pennsylvania. A copy of the marriage license evidencing the
fact is attached hereto and incorporated herein as "Exhibit B."
5. Petitioner has resided continuously in Cumberland County since 1989.
6. There are no judgments or decrees of record against Petitioner.
7. Petitioner now desires to have her name changed to Bonne Thomas
DuCharme.
WHEREFORE, Petitioner requests your Honorable Court to enter an Order
granting the Petitioner's request to change her name from Lavon Cheryl DuCharme to
Bonne Thomas DuCharme.
