FOR CITATION AND HEARING
AND NOW, this ~~l/- day of , 2009, upon consideration of
the foregoing petition, it is hereby ORDERED AND DECREED that a Citation be
awarded, directed to PATRICK R. SHUMATE to show cause why he should not be
adjudged an incapacitated person and plenary guardians of his estate and person be
appointed, returnable `rt~~~ 13 2009, at { I ; GZ~ o'clock, ~ M., prevailing
time.
The time and place of hearing on the petition for appointment of a guardian of the
estate and person of the alleged incapacitated person is fixed for ~~'~,,.~ ~3 ,
2009, at ~J_;_~bo'clock, ~M., prevailing time, before the Orphans' Court Division, at
the Cumberland County Courthouse, located at 1 Courthouse Square, Carlisle,
Pennsylvania.
At least twenty-days' written notice of the hearing shall be given to PATRICK R.
SHUMATE, the alleged incapacitated person, by serving him personally with a Citation
and this Order of Court and a copy of the foregoing Petition, together with an explanation
of the content and terms of the petition; and at least ~ days' written notice of the
petition and hearing shall also be given to the next of kin as set forth in the Petition,
either personally or by registered or certified mail.
