Defendants.
Defendant, Anthony D. Soustek, by and through his undersigned attorney, hereby
moves this Honorable Court for an Order compelling the Prothonotary of the Court of
Common Pleas of Cumberland County, Pennsylvania, to issue a subpoena duces tecum to
Rite Aid Pharmacy, c/o Rite Aid Legal Department, 30 Hunter Lane, Camphill,
Pennsylvania to produce medical records and medical bills regarding Amanda M.
Bowers, plaintiff in this matter. In support of his motion, Defendant states as follows:
This case arises from an automobile accident which occurred on October 6, 2006
and the plaintiff received medications from Rite Aid Pharmacy.
Wherefore, Defendant respectfully requests that this Court issue an order for the
issuance of a subpoena duces tecum by the Cumberland County Prothonotary compelling
Rite Aid Pharmacy produce medical records and medical bills of Amanda M. Bowers.
