If the Court finds you to be an Incapacitated Person, your rights will be affected, including our right to manage money and property and to make decisions. To be an incapacitated Person means that you are not able to receive and effectively evaluate information and communicate decisions and that you are unable to manage your money and/or other property, or to make necessary decisions about where you will live, what medical care you will get, or how your money will be spent. At the hearing, you have the right to appear, to be represented by an attorney, and to request a jury trial. If you do not have an attorney, you have the right to request the Court to appoint an attorney to represent you and to have the attorney's fees paid for you if you cannot afford to pay them yourself. You also have the right to request that the Court order that an independent evaluation as to your alleged incapacity. If the Court decides that you are an Incapacitated person, the Court may appoint a Guardian for you, based on the nature of any condition or disability and your capacity to make and communicate decisions. The Guardian will be of your person and/or your money and other property and will have either limited of full powers to act for you. If the court finds you are totally incapacitated, your legal rights will be affected and you will not be able to make a contract or gift of your money to other property. If the court finds that you are partially incapacitated, your legal rights will also be limited as directed by the Court. If you do not appear at the hearing (either in person or by an attorney representing you) the court will still hold the hearing in your absence and may appoint the Guardian requested. Petitioner June Heckman has had legal custody Jennifer Amy Heckman since a hearing of June 17, 1986 before Judge Sheely. A copy of the Order is attached hereto as Exhibit "A". In that capacity and in consultation with the Bancroft NeuroHealth multidisciplinary treatment team and staff of Cumberland Valley School District, June Heckman has been making decisions on behalf of Jennifer Amy Heckman regarding the medical and educational services which she receives. 7. Petitioner June Heckman has no adverse interest to Jennifer Amy Heckman. 8. Petitioner Patricia Sanderson has no adverse interest to Jennifer Amy Heckman. 9. Elaine M. Heckman, hereinafter "Mother" resides at 243 W. Columbia Road, Apt. 4 Enola, Pennsylvania and is the biological mother of Jennifer Amy Heckman. 10. Ronald Buda, hereinafter "Father", resides at 443 Hummel Avenue, Lemoyne, Pennsylvania, and is the biological father of Jennifer Amy Heckman. 11. Jennifer Amy Heckman has suffered from developmental disabilities since early childhood as well as various psychiatric diagnoses, and is presently receiving education in a residential setting where she requires 2:1 staff attention to address difficult behaviors including denuding and inappropriate voiding. 12. Jennifer Amy Heckman will be eighteen (18) years of age on December 29, 2002 and will continue to need a guardian to, inter alia, assist in medical and educational decision making and financial management. Ms. Heckman has a prepaid burial account with Americo/Rolling Green Cemetery, Camp Hill, Pennsylvania, in the amount of$7,369.40. 16. As of February 19, 2013, Ms. Heckman has debts, to the extent known, in the amount of$8,804.49 owing to the Commonwealth of Pennsylvania as payment for care and maintenance at Selinsgrove Center. 17. Jennifer Amy Heckman was adjudicated an incapacitated person by Order of this Court dated August 7, 2002 and a plenary guardian of the estate and person were appointed. 2. Patricia Sanderson successfully served as Plenary Guardian of the Estate, and is hereby discharged as Guardian of the Estate of Jennifer Amy Heckman. 3. The Court finds that the interest of Jennifer Amy Heckman would be served by the appointment of a Successor Guardian of her Estate and hereby appoints the Guardian Office at Selinsgrove Center, and such other Guardian Offices at state facilities as become appropriate, as Successor Plenary Guardian of the Estate. The Guardian Office is hereby authorized to receive, hold and dispose of the estate of Jennifer Amy Heckman as the duly appointed Guardian of the Estate under Court Decree. It is established that Ms. Heckman has sufficient means for her burial. Therefore, the Guardian Office is not required to establish an additional burial fund. 4. Patricia Sanderson shall remain as Plenary Guardian of the Person of Jennifer Amy Heckman with authorization to make decisions on Ms. Heckman's behalf i concerning her medical care and treatment, educational placements, and admission to facilities for the care and treatment of persons with intellectual disabilities, as well as to consent to and authorize medical treatment as recommended by her treating physicians. 5. Jurisdiction over all guardianship matters relating to Ms. Heckman are hereby transferred to Snyder County. 6.
