Keystone Guardianship Services was appointed Guardian for Janette Neely, an alleged incapacitated person by the Cumberland Country Orphan's Court by Order dated March 5th 2008 (see copy of Order attached as Exhibit "1"). 2. Janette Neely is presently a resident of Country Meadows located at 951 Sandhill Road, Hershey, Dauphin County Pennsylvania 17033. 3. Keystone Guardianship Services has been working cn selling the real estate owned by Janette Neely in order to have funds readily available for her personal and medical care, as well as continual real estate maintenance. Page 1 of 3 Pages Estate Janette Neely An Incapacitated Person 4. Janette Neely is the sole owner of a piece of real estate known as 3090 Spring Road, Middlesex Township, Cumberland County, Pennsylvania (Parcel No 21-13-0966-018) (See copy of the Deed attached as Exhibit "2"). 5. Said Real Estate was appraised by Diversified Appraisal Services, 35 East High Street, Suite 101, Carlisle, Pa 17013 for Ninety Two Thousand ($92,000.00) Dollars. (See copy of the appraisal attached as Exhibit "3"). 6. Said Real Estate is assessed at $109,660.00 X 1.22 bringing the assessed value of the real estate r_o $133,785.20(see copy of the assessment information attached as Exhibit "4°). 7. Said house is in dire need of repairs and renovations. 8. This property was purchased by Janette Neely on or about August 1998 for the consideration of $71,500.00. There i.s presently a mortgage on the said real estate. A mortgage in the amount of $55,000 was taken out on this property. There is a current balance of $22,980.76, and a monthly payment of $464.13. (see copy of the current Mortgage Information attached as Exhibit "5°). Page 2 of 3 Pages Estate _ Janette Neely An Incapacitated Person 9. The Guardian's first attempt to sell the property will be through sealed bids "AS IS" for no less than 10% (t;en percent) of One Hundred Twelve Thousand Five Hundred ($112,500.00) Dollars (the average of the appraised value of $92,000.00 and current assessed value of $133,000.00) (see copy of the proposed advertisement of the bids attached as Exhibit "6") . The sealed bid process will be used to sell the property without the cost of auctioneer or real estate commissions. WHEREFORE, the aforementioned guardian respectfully requests this Honorable Court enter an Order directing the sale of 3090 Spring Road, Middlesex Township, Cumberland County, Pennsylvania (Parcel No 21-13-0966-018) owned by Janette Neely "AS IS" for no less than 10°s (ten percent) of One Hundred Twelve Thousand Five Hundred ($112,500.00) Dollars. March 5, 2008:- Janette Neely, age 84, Is currently a resident at Arden Court, Harrisbur®, Pennsylvania, a facility specializing in the care of Alzheimer patients. We find that duo to her Alzheimer di~eas®, her ability to receive and evaluate Information effectively and communicate decision are Impaired to such a significant extent that she is unable fo manage herfinanciai resources and unable to meet essential requirements for her phys(cai heath and safety. Her prognosis is for furthermental~ deterloratlon. We find that she is an incapacitated person in need of a plenary guardian of her persori and .plenary guardian of her estate for an Indefinite period.
