surviving spouse is 3% [72 P.S. {}9116 (a) (1.1) (i)].
For dates of death on or after January 1, 1995, the tax rate imposed on the net value of transfers to or for the use of the surviving spouse is 0%
[72 P.S. {}9116 (a) (1.1) (ii)]. The statute does not exempt a transfer to a surviving spouse from tax, and the statutory requirements for disclosure
of assets and filing a tax return are still applicable even if the surviving spouse is the only beneficiary.
For dates of death on or after July 1, 2000:
The tax rate imposed on the net value of transfers from a deceased child twenty-one years of age or younger at death to or for the use of a natural
parent, an adoptive parent, or a stepparent of the child is 0% [72 P.S. {}9116 (a) (1.2)].
The tax rate imposed on the net value Df transfers to or for the use of the decedent's lineal beneficiaries is 4.5%, except as noted in 72 P.S. {}9116

The tax rate imposed on the net value of transfers to or for the use of the decedent's siblings is 12% [72 P.S. {}9116 (a) (1.3)]. A sibling is defined,
under Section 9102, as an individual who has at least one parent in common with the decedent, whether by blood or adoption.

I, DONALD EUG1~IE KECK, a legal resident of Middlesex Township,
Cumberland County, Pennsylvania, being of sound and disposing mind, memory
and understanding, do hereby make, publish and declare this as and for my
last Will and Testament, hereby revoking all other wills and codicils
heretofore made by me.
FIRST: I direct that all my just debts and funeral expenses,
including my grave marker, shall be paid from the assets of my estate as
soon as practicable after my decease.
S~C[~D: I direct that all taxes that may be assessed in consequence
of my death, of whatever nature and by whatever jurisdiction imposed, shall
be paid from my residuary estate as a part of the expense of the
administration of my estate.
~]I~D: I devise and bequeath the residue of my estate, of every
nature and wherever situate, to my Wife, CYNTHIA A. KECK, providing she
shall survive me by thirty (30) days.
~: Should my Wife, CYNTHIA A. KECK, predecease me or die on or
before the thirtieth day following my death, I devise and bequeath my
businesses known as "K Street Video" and "North End Shoe Repair" (both
businesses being operated under the same federal employer identification
number) to my Daughter, LISA M. KECK. Should both my Wife, ~ A. KECK
and my Daughter, LISA M. KECK, predecease me or die on or before the
thirtieth (30th) day following my death, I devise and bequeath said
businesses to my Daughter, NICOLE R. KECK. In the event that she is a minor
at the time of my death, I request that my trustees hereinafter named
operate said businesses for her benefit as part of the trust hereinafter
est~_blished. It is my desire that said businesses, to the extent possible,
be preserved intact as part of the assets of said trust and that full
ownership of the businesses be turned over as a going concern to my
Daughter, NIOOLE R. KECK at the time she attains the age of 21. In the
event that my wife and both daughters aforesaid predecease me or die on or
before the thirtieth (30th) da~v following my death, th~n I devise and
bequeath said businesses to MR. and MRS. GIENNWOOD BICK~A, or the survivor.
FIF/~: Should my Wife, CYNTHIA A. KECK, predecease me or die on or
before the thirtieth day following my death, I devise and bequeath the
residue of my estate, of every nature and wherever situate, to my children,
equally, provided that the share of any child who predeceases me or dies on
or before the thirtieth day following my death shall be distributed to his
or her issue, per stirpes, living on the thirty-first day following my
death, and in default of any such then-living issue, such share shall be
added to the share or shares for my other children.
SIXI~: I direct that the share of any beneficiary under the age of
21 years shall be held, IN TRUST, however, by MR. and MRS. GLENNWOOD BICKTA,
as Co-Trustees, or the survivor, to hold said share for the benefit of each
said beneficiary under the age of 21, upon the following terms and
conditions:
A. To pay the'income and so much of the principal as may, i~
the. sole discretion of my Trustee, be necessary for the
maintenance, support, medical expenses and education of each
beneficiary.
B. The amount to be paid for the benefit of any of said
beneficiaries shall be determined frc~ time to time by the
n~ of each of said beneficiaries, and the amounts and times
of said payments shall be determined by such need. The said
payments may be made by my Trustee directly to each of the
said beneficiaries, or to such of them as may be, in the sole
opinion of my Trustee, of such age and ability to handle
properly the funds so paid to such beneficiary, or may be made
by my said Trustee directly to the person having the custody
and care of any of the said beneficiaries, or may be made by
my said Trustee directly to any institution entitled to such
payment by reason of services rendered or to be rendered to
any of the said beneficiaries.
C. To pay the accumulated ~ and principal then remaining
in their hands to the said beneficiaries, upon each
beneficiary's attaining the age of 21 years.
D. Any and all payment or payments of any sum or sums,
whether in cash or in kind, and whether for principal or
income, payable to said beneficiaries, shall be made upon the
sole receipt of the respective beneficiary to whom the payment
is made, and free f-,-cm-~ anticipation, alienation, assignment,
attachment and pledge, and free from control by the creditors
of any such beneficiary. All shares of principal and income
herein given shall be free from anticipation, assignment,
pledge or obligation of any beneficiary, and shall not be
subject to any execution or attachment.
E. In the event of the renunciation, death, resignation or
inability to act for any reason whatsoever of the said MR. and
MRS. GTFNNWOOD BICK~A, I nominate, constitute and appoint MR.
and MRS. ARTHUR S. KECK as Co-Trustees, or the survivor, under
the terms and conditions of this Paragraph Fourth.
SEV~q~: Should my Wife, CYNTH/A A. KECK predecease me or die on or
before the thirtieth day following my death, I nominate, constitute and
appoint MR. and MRS. GLENNWOOD BIC~A, or the survivor, ~p,a~dians of the
person of my Daughter, NIODLE R. KECK. In the event of the renunciation,
death, resignation or inability to act for any reason whatsoever of the said
MR. and MRS. GTRNNWOOD BICKTA, I nominate, constitute and appoint MR. and
MRS. ARTHUR S. KECK, or the survivor, as guardians as aforesaid.
EIGHTH: I nominate, constitute and appoint my Wife, CYNTHIA A.
KECK, Executrix of this, my Last Will and Testament. In the event of the
renunciation, death, resignation or ir~hility to act for any reason
whatsoever of the said CYNTHIA A. KECK, I nominate, constitute and appoint
MRS. GLENNWOOD BIC~TA, Executrix of this, my Last Will and Testament. I
hereby relieve my Executrix or her successor frc{n the necessity of posting
security in connection with their duties as such in any jurisdiction in
which they may be called upon to act, insofar as I am able by law so to do.

A refund of a tax credit, which was not requested on the Tax Return, may be requested by completing an "Application
for Refund of Pennsylvania Inheritance and Estate Tax" (REV-I$13). Applications ara available at the Office
of the Register of Rills, any of the 23 Revenue District Offices, or by calling the special 24-haur
ansaering service for forms ordering: 1-800-36Z-Z050~ services for taxpayers with special hearing and / or
speaking needs: 1-800-447-30Z0 (TT only).
Any party in interest not satisfied with the appraisement, allowance, or disallowance of deductions, or assessment
of tax (including discount or interest) as shown on this Notice must ob[act within sixty (60) days of receipt of
this Notice by:
--written protest to the PA Department of Revenue, Board of Appeals, Dept. 281021, Harrisburg, PA 171ZB-IOZZ, OR
--election to have the matter determined at audit aT tho account of the personal representative, OR
--appeal to the Orphans' Court.

If any tax due is paid within three (3) calendar months after tho dacedent's death, a five percent (DX) discount of
the tax paid is allowed.
The 152 tax amnesty non-participation penalty is computed an tho total of the tax and interest assessed, and not
paid before January 18, 1996, the first day after the and of the tax amnesty period. This non-participation
penalty is appealable in the same manner and in the the same time period as you would appeal the tax and interest
that has been assessed as indicated on this notice.
Interest is charged beginning with first day of delinquency, or nine (9) months and one (1) day from the date of
death, to the date of payment. Taxes which became delinquent before January 1, 1982 bear interest at the rate of
six (62) percent per annum calculated at a daily rate of .000164. All taxes which became delinquent on and after
January 1, 1982 will bear interest at a rata which will vary from calendar year to calendar year with that rate
announced by the PA Department of Revenue. The applicable interest rates for 1982 through Z004 are:
