of Attorney dated Nlay 11, 1977, which power has not been revoked, disclaim my
interest as heir at law of the said decedent. I hereby declare that I have not
(1) taken possession or accepted delivery of any property or interest as such heir
at law; (2) waived the right to disclaim; (3) assigned, conveyed encumbered,
pledged or other~rvise transferred such intE~rest or contracted to do so, and that I
have not been adjudicated an imcompetent.
