FINAL DECREE
AND NOW, this 1 l day of ~ ~~ , 2009, based upon the evidence received
and the record, this court finds by clear and convincing; evidence, that BERTHA E. REESE

suffers a disability that totally impairs ~ capacity to receive and evaluate information

effectively and to make and communicate decisions concerning 1£~s management of financial

affairs or to meet essential requirements for l~physical health and safety.
DAVID L. REESE is hereby appointed plenary permanent Guardian of the PERSON OF
BERTHA E. REESE. The Guardian of the PERSON shall file a report on the social, medical
and other relevant conditions as required by 20 Pa. Cons. Stat. Section 5521(c).
DAV[D L. REESE is hereby appointed plenary permanent Guardian of the ESTATE OF
BERTHA E. REESE. The Guardian of the ESTATE shall file a report within 90 days and
annually thereafter. These reports shall comply with ZO Pa. Cons. Stat. Section 5521(c).
DAVID L. REESE shall not be required to post bond.
The (JUardian of the PERSON shall have authority and responsibility to decide where
BERTHA E. REESE shall live and how meals, personal care, transportation and recreation will
be provided. The Guardian of the PERSON shall have the authority to authorize and consent to
medical treatment and surgical procedures necessary for the well being of BERTHA E. REESE.
The Guardian of the ESTATE shall have authority and responsibility to manage and use

BERTHA E. REESE's property solely for ~e benefit in accordance with 20 Pa. Cons. Stat.
Section 5536(x).
