MALESICH, files this Mechanic's Lien Claim pro se, pursuant to 49 P.S. 1503, based upon the following: 1. Claimant, Dustin J. Malesich, is a sole proprietor with a mailing address of 730 South Tenth Street, Harrisburg, Pennsylvania 17104 and files this Mechanic's Lien Claim as a contractor, since Claimant, had direct dealings with the owner of the property, Joshua Kesler. 2. The aforesaid owner or reputed owner, is Joshua Kesler , an adult individual with a mailing address of 245 North Twenty Fifth Street, Camp Hill, Pennsylvania, 17011. 3. Joshua Kesler has represented to Claimant that he is the owner of the property in question, owns an estate or legal interest therein, and has entered into an Authorization with claimant. A true and correct copy of the aforesaid Authorization is attached hereto, mazked as Exhibit `A' and incorporated herein by reference. 4. The date on which Claimant began work for which claim is made was on or about April 15th, 2008. 5. This claim is made for property rehabilitation work to 245 North Twenty Fifth Street, in the Borough of Camp Hill, Cumberland County, Pennsylvania 17011 as specified in the Authorization attached hereto, marked as Exhibit `A' and incorporated herein by reference. 6. The amount claimed is One Hundred and Twenty Six Thousand and Nine Hundred and Seventy Eight Dollars ($126,978.00), for materials and labor, plus cost of collection including reasonable attorney's fees. The Claimant has no note or other collateral security for its claim. 7. The improvement and property which is subject to the lien is known as 245 North Twenty Fifth Street, in the Borough of Camp Hill, Cumberland County, Pennsylvania 17011 . MALESICH CONTRACTING SERVICES 730 South Tenth Street Harrisburg, Pennsylvania 17104 717-919-7827 CONTRACT OF SERVICES 4112/08 This contract is made between Joshua Kesler, owner of property 245 North Twenty Fifth Street, Camp Hill, Pennsylvania, 17011, and Dustin J. Malesich, for the rehabilitation of the owner's said premises and shall serve as an Authorization of following work to commence. First Floor -removal of wet plaster caused from persistent roof leak from living room walls and chimney. Strip framing from walls to remove mold. Remove and repoint crumbling masonry joints on the interior and exterior of brick and stone chimney wall unit. -replace plaster ceilings in kitchen and dining room with drywall. Remove wallpaper and other coverings to mitigate mold on kitchen, dining room, hallway walls. Paint kitchen, dining room, and den. -resurface plaster walls in kitchen and den. -partially remove chipping lead paint in exterior doorways and repaint to prevent fwther deterioration. -replace kitchen faucet and drain tines, install new dishwasher. -remove moldy flooring in den and replace with new carpet and padding. Second Floor -Remove oak floorboards down to the sub floor in the hallway and bedroom, approximately 400 square feet, and replace with'/," plywood sub floor, padding and carpet. Customer may select carpet and color from apre-selection by contractor. -Strip out all wet plaster, trim, and doors in the room ruined by roof leak. -Patch walls and ceilings in hallways. -Remove plaster and mold from bathroom ceiling, Hang, tape and finish drywall on bathroom ceiling and paint.. -Paint bathroom, hallway, and two bedrooms. The other two bedrooms will remain inhabitable at customer`s request. -Repaire faucet fn the operable bathroom that is to have the drywall replaced. Basement -mitigate pervasive mold condition throughout basement. painting ceiling, walls, and floor in part of the basement. -run ground wire from electric meter base to water shut off valve, which i required by PPL to restore electric service to property. Sebastian Electric will hang fallen wires and weather head to meter. -JAG Services HVAC will service gas burners and hot water boiler. Exterior Remove several tons of yard waste and debris. Make all improvements suggested by the Cumberland County Redevelopment authority. Remove Locust trees deemed by Stoner's Tree Service to be an immediate threat to the property (six removal, four toppings). The total cost of above improvements shall be $126 978.00 All checks shall be made payable to Dustin Malesich.
