The Petitioner Daniel Lee Zechman resides at 168 Spring Road, Carlisle, Cumberland County, Pennsylvania. 2. The Petitioner Venessa Jane Henry resides with Madisan Elizabeth Henry (hereinafter "Madisan") at 520 Longs Gap Road, Apartment B, Carlisle, Cumberland County, Pennsylvania. 3. The Petitioners are the natural parents ofMadisan, a minor, whose date of birth is October 1, 1999. 4. In addition to Madisan's present address, since birth, Madisan has resided with either or both of the Petitioners at the following addresses: a. 6 Wilbert Drive, North Middleton, Cumberland County, Pennsylvania, from 2002 to 2003; b. 104 Lincoln Street, Carlisle, Cumberland County, Pennsylvania, from 2000 to 2002; and c. 158 West North Street, Carlisle, Cumberland County, Pennsylvania, from 1999 to 2000. 5. Madisan was born out of wedlock and was bom as Madisan Elizabeth Henry. 6. The Petitioners and Madisan have continuously resided in Cumberland County, Pennsylvania, since Madisan's birth in 1999. 7. Petitioners desire and feel it would be in the best interest of Madisan that her surname conform to the surname of her natural father, Petitioner Daniel Lee Zechman. 8. Petitioners do not wish to change Madisan's name for any illegal purpose, to avoid payment of taxes or other debts, or to avoid any obligation. 9. Petitioners' cotmsel, Sean M. Shultz, Esquire, has searched the index in the Prothonotary's Office in the Court of Common Pleas of Cumberland County and there are no judgments or decrees of like character of record against Madisan Elizabeth Henry. WHEREFORE, the Petitioners, Daniel Lee Zechman and[ Venessa Jane Henry request this Honorable Court enter a decree changing the name of Madisan Elizabeth Henry to Madisan Elizabeth Zechman.
