Luta by and through her undersigned attorney, Joseph L. Hitchings, Esquire, and avers in support of her Petition For Change of Name of her minor daughter, Alessandra Elzbieta Somma as follows: 1. Petitioner, Tasha N. Luta is an adult individual residing at 102 Hearthstone Court, Dillsburg, Pennsylvania 17019. 2. Petitioner is the natural mother of Alessandra Elzbieta Somma, who was born on December 23, 2007, in Harrisburg, Dauphin County, Pennsylvania. A true and correct copy of her birth certificate is attached hereto as Exhibit "A". 3. The minor was named Alessandra Elzbieta Somma at birth, and as of the date of this Petition, the minor has always been known by that name. 4. The minor resides with her natural mother, Petitioner, Tasha N. Luta, at 102 Hearthstone Court, Dillsburg, Pennsylvania 17019, and has lived at this address since birth. 5. The natural father of the minor is Andrew J. Somma, an adult individual who resides at 125 Long View, Carlisle, Cumberland County, Pennsylvania 17103. 6. Petitioner seeks to change the minors name from Alessandra Elzbieta Somma to Alessandra Elzbieta Luta, so that the minor will have the same last name as her mother, easing school enrollment and preventing possible emotional confusion with having a different last name. 7. The minor does not have any outstanding judgments, has never been convicted of a crime, and is not involved in any pending legal actions, with the exception of an open child support order in Cumberland County Domestic Relations office. Petitioner requests the Court waive the requirement for official name and judgment searches. 8. The minor does not have any outstanding support obligations against her. 9. The minor's father, Andrew J. Somma, has not initiated visitation rights with the child since her birth, has not made any attempts to contact the minor and consents to the requested name change. A true and correct copy of father's Affidavit of Consent to the Name Change is attached hereto and marked as Exhibit "B" WHEREFORE, Petitioner, Tasha N. Luta respectfully requests that this Honorable Court enter an Order changing her minor daughter's name from Alessandra Elzbieta Somma to Alessandra Elzbieta Luta.
