BOURINSKI, Petitioners Civil Action - Law PETITION FOR NAME CHANGE. AND NOW, comes Austin S. Bourinski, and Celeste N. Bourinski, by and through their Attorney, Jane Adams, Esquire, and respectfully requests this Honorable Court to change their names for the following reasons: 1. Petitioners, Austin S. Bourinski and Celeste N. Bourinski, (hereinafter referred to as "Petitioners") currently live at 53 S. Bedford Street, Carlisle, Cumberland County, Pennsylvania, 17013. 2. Petitioner Celeste N. Bourinski is twelve (12) years old. Her date of birth is October 22, 1996. 3. Petitioner Austin S. Bourinski is thirteen (13) years old. His date of birth is July 7, 1995. 4. Since 2001, Petitioners have lived at 53 S. Bedford Street, Carlisle, Pennsylvania, with their natural Father, Brian McMullan, and their brother, Brandon McMullan, who is fourteen years old. 5. Petitioners' Mother, Lynn M. Bourinski, died on October 12, 2002. 6. Petitioners wish to change their names to: Austin S. McMullan and Celeste N. McMullan. 7. Petitioners want to use the last name of their natural Father because it would be less cumbersome and they feel more comfortable having the same last name as their natural Father and brother. 8. Petitioner believes that using their natural Father's last name would avoid confusion regarding school and medical records. 9. Petitioner's Father joins in this petition and has reviewed the information in this petition; his verification is attached. 10. Petitioner is asking that notice to natural Mother be waived due to the fact she is deceased; an original death certificate will be requested and presented at the hearing. 11. Petitioners are under eighteen (18) years old and have no outstanding credit debt that could be avoided through a name change. 12. Petitioners have no outstanding federal or state tax debt that could be avoided through a name change. WHEREFORE, Petitioner requests that this Honorable Court Order change their name.
