PETITION FOR CHANGE OF NAME TO THE HONORABLE JUDGES OF SAID COURT: AND NOW comes James P. Sheppard, Esquire, attorney of record for Sally Ann Fennessy and represents the following: 1. Petitioner is Sally Ann Fennessy, an adult individual, who resides at 624 Gates Lane, Enola, Cumberland County, Pennsylvania 17025. 2. Petitioner's date of birth is February 14, 1945. 3. Petitioner has resided at the following addresses for the five (5) years previous to the filing of this Petition: a. 624 Gates Lane, Enola, Cumberland County, PA 17025 4. Petitioner desires and intends to change her name to Sallianne Fennessy. 5. Petitioner requests that her name be changed because as an adult, she has always used the name of "Sallianne Fennessy" as her known name. That is the name that appears on her Pennsylvania Driver's License. That is the name which appears on her bank accounts, her creditor accounts, and on all records known to her with the exception of her Pennsylvania Birth Certificate and her Social Security Card. 6. Petitioner recently lost her Social Security Card and applied for a new card. At the time she applied for the new card, she requested that her name appear as "Sallianne Fennessy" on the new card. She was denied the request and was A. denied the issuance of a new card because she does not have any photo identification which would prove her legal name "Sallianne Fennessy" to allow the issuance of a replacement card. Petitioner is without a Social Security Card or proof of Social Security Number. 7. The proposed change in Petitioner's name, if granted, will not be detrimental to the interests of any other person and is not against public interest. The Petition is not intended to defraud creditors. 9. The name change, if granted, will actually eliminate any potential confusion as to her identity by allowing her used alias name to become her actual legal name. WHEREFORE, Petitioner requests the Court, pursuant to the Act of December 16, 1982, P.L. 1309, No. 295 Section 2, 54 Pa.C.S. Section 701, for the following: a. to fix a hearing on this Petition; b. to direct that notice be given on this Petition and hearing; and c. to enter a decree changing the Petitioner's name from Sally Ann Fennessy to Sallianne Fennessy.
