Do you contemplate a change in placement in the near future?
No.
Describe major mental or medical conditions of the incapacitated person.
No change.
Describe social and recreational activities.
Claire participates in social activities sponsored by CPARC. She is
currently on a "swim team," attends practices and competitions.
Medical and psychological services.
Claire now has medical insurance through her place of employment.
Describe hospitalization or medical treatment since last report.
Claire was taken to the emergency room by CPARC staff for stomach
pains. Tests were inconclusive. She has since had further testing done
and no medical cause was found. We will continue to observe.
How often do you visit?
Once a month. More frequently, Claire comes to my house with her
roommate for dinner and to spend time with me, her mother, and her two
sisters.
Are there needs that are not being provided?
No.

Continued.
Do you have concerns about Claire's physical or mental well being or finances

Not at this time.

Specifically set forth amounts and sources of income.
Claire continues to be employed by Country Meadows Retirement Home,
earning approximately $8 per hour. Income varies with number of hours
worked. Paychecks are direct deposited in Claire's checking account, to
which only I have access and from which I pay her bills. These include
CPARC rent of $312 per month and Comcast bill of $53 per month. I
transfer money to an account which Claire can access with an ATM card,
and from which she pays for groceries and incidental expenses, with the
assistance of CPARC staff.
Expenditures since last report.
Approximately $1,000 for legal expenses related to her trusts, to determine
if there is a better method of handling that money. This process presently
underway, and any future legal expenses will be paid from the trust, with
the approval of the trustees. This was taken from income from the Claire
Wong Crego Trust.
Approximately $2,000 for living room furniture for her apartment. (She
took Bedroom furniture from home) This was taken from Claire's
checking account.
$800 for a laptop computer. This was taken from income from the Claire
Wong Crego Trust.
Was any principal used without obtaining written court approval?
No principal was used.
Is there sufficient income for expenses?
Yes.
