That your Petitioner is Jessica Noelle Smith, an adult individual, who resides at 52 Courtyard Drive, Carlisle, Cumberland County, Pennsylvania. 2. That your Petitioner is an adult individual over the age of 18. 3. That your Petitioner was a resident of Pennsylvania from birth to four years of age and more recently since March 11, 2009. 4. That your Petitioner is single. 5. That your Petitioner desires to change her name back to her birth name, "Miller," as she was adopted by her step father. 6. That your Petitioner has no fraudulent purpose in changing her surname, having no creditors nor judgments, decrees or other process outstanding against her. 7. There has not been a judge previously assigned to this matter. 8. There is no other party or counsel for a party in this matter to be consulted with in regard to concurring in this matter.
