The Plaintiffs are Dale C. Judy and Jo E. Judy, his wife, adult individuals who reside at 207 Conodoguinet Avenue, Apt. 2, Camp Hill, Pennsylvania 17011. 2. The Defendants are Ashley R. Donmoyer, an adult individual who resides at 233 S. Main Street, Marysville, PA 17053 and John M. McKee, an adult individual who resides at 454 High Street, West Fairview, Pennsylvania. 3. Plaintiff seeks custody of the following child: Name Present Address Aae Jade Phoenix Tidd 207 Conodoguinet Ave., Apt 2 Born 3;7/2009 Camp Hill, PA 17011 The child was born out of wedlock. The child is presently in the custody of Plaintiffs. Donmoyer, currently residing at 233 S. Main Street, Marysville, Pennsylvania 17053. She is single. The father of the child is believed to be John M. McKee, currently residing at 454 High Street, West Fairview, Pennsylvania. He is single. 4. The relationship of Plaintiffs to the child is: Plaintiff Dale C. Judy is the child's great uncle. The Plaintiffs currently reside with the following persons: Name Relationshiy Jade Phoenix Tidd Dale Judy' s great ~epl~-•n +eC~ Stephen Good Jo Judy's son-in- law 5. The relationship of Defendant Ashley R. Donmoyer to the child is that of natural mother. Defendant John M. McKee is believed to be the child's natural father. 2 Defendant Ashley R. Donmoyer currently resides with the following persons: Name Jonathan T. Tidd Patsy K. Tidd John S. Tidd Relatioashiy Ashley R. Donmoyer's boyfriend Jonathan T. Tidd's grandmother Jonathan T. Tidd's grandfather 6. Plaintiffs have not participated as~a party or witness, or in any other capacity, in other litigation concerning the custody of the child in this or another court. Plaintiffs have no information of a custody proceeding concerning the child pending in a court of this Commonwealth or any other state. Plaintiffs do not know of a person not a party to the proceeding who has physical custody of the child or claims to have custody or visitation rights with respect to the child. 7. The best interest and permanent welfare of the child will be served by granting the relief requested because the natural parents are unable to care for the c~iild, and Plaintiffs have the ability and desire to provide a stable and nurturing environment for the child. 8. Each parent whose parental rights to the child have not been terminated and the person who has physical custody of the child have been named as parties to this action. There are no other persons who are known to have or claim a right to custody or 3 visitation of .the child to be given notice of the pendency of this action and the right to intervene. WHEREFORE, Plaintiffs request the court to grant custody of the child, with reasonable visitation rights to the natural mother.
