Petitioner Michelle Suk Yee smith, requests that this Court order a change of the
petitioner's name, pursuant to 54 Pa. Cons. Stat. Section 701, _eJ 1M., and in support thereof
makes the following the averments:

2. During the five years preceding the filing of this petition, the petitioner has resided

I am already known by my proposed name in my profession and I would like to use this name in all my
personal and business dealings.
6. The Petitioner has not been convicted of a felony.
WHEREFORE, the petitioner respectfully requests that this Court enter an Order
changing his/her name from Michelle Suk Yee Smith to uk Yee Smith

VERIFICATION
I, Michelle Suk Yee Smith , verify that the statements made in the
foregoing Petition are true and correct to the best of my knowledge or information and
belief. I understand that this verification is made subject to the penalties of 18 Pa.C.S.
§ 4904 relating to unsworn fabrication to authorities, which provides that if I knowingly
make false averments, I may be subject to criminal penalties.
