Petitioner has been a resident of the Commonwealth of Pennsylvania for at least six (6) months preceding the filing of this Petition. 8. Petitioner desires to change her name from Danielle McCann to Danielle McCann Vigilante. PETITION FOR NAME CHANGE AND NOW, this 7th day of May, 2004, comes the Petitioner, Danielle McCann, by and through her attorneys, Saidis, Shuff, Flower & Lindsay, and files the following Petition, and in support thereof avers as follows: 1. Petitioner, Danielle McCann, is an adult individual residing at 152 South West Street, 2nd Floor, Carlisle, Cumberland County, Pennsylvania. Petitioner has resided at the above-referenced address for approximately four SAIDIS SHUFF, FLOWER & LINDSAY 9. Petitioner's chosen surname, Vigilante, is her mother's maiden name. Petitioner's mother and father were divorced in February 2003 and Petitioner's mother re- took her malden surname. 10. Additionally, Petitioner has not had any contact with her natural father in excess of twelve (12) years. 11. Petitioner now desires to legally take the surname of Vigilante, thereby legally changing her name to Danielle McCann Vigilante. 12. Petitioner does not wish to change her name for any illegal purpose or to avoid any obligation(s). Petitioner has been fingerprinted by the Pennsylvania State Police and her fingerprint card is attached hereto as Exhibit "A" and is by reference incorporated herein. 13. Petitioner has never been convicted of a felony. 14. Petitioner's Social Security number is 206-60-5717.
