AND FIDUCIARY CODE TO ADJUDICATE WALTRAUD G. WYLIE TO BE INCAPACITATED
AND TO APPOINT A PLENARY GUARDIAN
PRELIMINARY DECREE

annexed Petition, it is ORDERED AND DECREED that a hearing on this matter is set for the ~7`~day of
~__ 2009, in Courtroom No. ~ at ~ ~~~~ o'clock ~ .M. at the
Cumberland County Courthouse, One Courthouse Square, Carlisle, Pennsylvania, and that a Citation be
issued to Waltraud G. Wylie commanding her to show cause why she cannot appear at the aforementioned
hearing pursuant to the Petition of Holy Spirit Hospital of the Sisters of Christian Charity to have Waltraud G.
Wylie adjudicated an incapacitated person and to have Brian Brooks d/b/a the Pennsylvania Guardianship
Association appointed as the plenary guardian appointed for her person and her estate. Notice of the
hearing shall be given to Waltraud G. Wylie in accordance with 20 P.S. § 5511(a) not less than twenty (20)
days prior to the hearing.
