Petitioner Megan Irene Morales, requests that this Court order a change of the
petitioner's name, pursuant to 54 Pa. Cons. Stat. Section 701, g OZ., and in support thereof
makes the following the averments:

5. The reasons for the requested change of name are
I am filing for divorce and my husband does not want me to keen the last name Morales I would like to
change my last name to my mother's maiden name.
6. The Petitioner has not been convicted of a felony.
WHEREFORE, the petitioner respectfully requests that this Court enter an Order
changing his/her name from Megan Irene Morales
to Megan Irene Anderson
