I, Elizabeth P. Mullaugh, hereby depose and state as follows:
1. On May 6, 2009, I personally served Georgia D. Johnstone, the alleged incapacitated
person, with the Citation and Order of the court dated April 21, 2009, as well as a copy of the
Petition filed April 20, 2009 petitioning the Court for an appointment of plenary Guardian of the
Person and Estate of Ms. Johnstone, together with an explanation of the contents and terms of the
Citation and Petition.
2. Personal service was made on Ms. Johnstone at The Oaks at Bethany Village,
Mechanicsburg, Pennsylvania.
