PETITION FOR APPOINTMENT OF GUARDIAN
TO THE HONORABLE JUDGES OF SAID COURT:
The petition of John H. Borkenhagen and Diana Borkenhagen respectfully
represents that:
1. John H. Borkenhagen and Diana Borkenhagen aze husband andcwife and o

3. Hannah Borkenhagen resides with both parents at 431 Hogestown Road, ~
Mechanicsburg, PA 17050.
4. Hannah Borkenhagen was left $16,605.52 dollazs in the form of Life
Insurance benefits, left to her by Robert J. Ritger (hereinafter life
insurance benefits).
5. Robert J. Ritger died on August 12, 2007.
6. The Decedent, Robert J. Ritger, was the foster pazent of Petitioner, John H.
Borkenhagen.
7. The interest of Hannah Borkenhagen should be represented in this matter
by her natural pazents John H. Borkenhagen and Diana Borkenhagen.
8. The monies received from Robert J. Ritger are presently being held by
Northwestern Mutual Contract No. 6988969_
9. No other guazdian has been previously appointed for Hannah
Borkenhagen.
10. John H. Borkenhagen and Diana Borkenhagen upon consultation with their
fmancial planner from Edwazd Jones wish to have said proceeds
deposited in an Edwazd Jones Custodial Account invested in Mutual
Funds further identified as Account Number 242-10417.
WHEREFORE, Petitioners John H. Borkenhagen and Diana Borkenhagen
respectfully request that they be appointed as guazdians for Hanna Borkenhagen, a
minor, for purposes of investing the insurance benefits in accordance with this
petition.
