PETITION FOR CHANGE OF NAME TO THE HONORABLE JUDGES OF THE COURT: The Petition of Danielle E. Morrill, filed by and through her counsel, Linda A. Clotfelter, Esquire, respectfully sets forth the following: 1. Petitioner is Danielle E. Morrill, an adult individual who resides at 7 Ash Drive, Mechanicsburg, Cumberland County, Pennsylvania 17050. 2. Petitioner is the parent of one minor child, namely, Allison E. Clites, (hereinafter the "Child"), age 14, having been born July 7, 1994. The Child resides with Petitioner at 7 Ash Drive, Mechanicsburg, Cumberland County, Pennsylvania 17050. 3. The Child's natural father is Keith S. Clites, an adult individual whose last known address is 104 Third Street, Boiling Springs, Cumberland County, Pennsylvania 17008. He is Petitioner's former husband. 4. Petitioner is remarried and as such the Child has a stepfather, Daron G. Morrill. He also resides at 7 Ash Drive, Mechanicsburg, Cumberland County, Pennsylvania 17050. 5. Petitioner believes it is in the best interests of the Child to have her named changed to Allison E. Morrill for the following reasons: a. The Child has expressed a very strong desire to change her last name to "Morrill" which is the surname of her stepfather, Daron G. Morrill. b. The request for the name change was initiated by the Child. C. The Child is very close to her stepfather who has been an active, loving, and stable influence in her life for more than nine (9) years. d. Petitioner and her husband both support the Child in her desire to seek a name change and Petitioner believes that the Child's strong desire to change her name has not been requested to hurt the Child's natural father, but is motivated by the Child's goal of having a bond with her stepfather. e. Petitioner has explained to the Child the legal ramifications of the name change including but not limited to the possibility that her natural father may oppose the name change. f. Despite those concerns, the Child has remained adamant in her desire to change her name. g. The Child has not had any contact with her father for more than a year and it has been several years since her father has been truly involved in her life. Upon information and belief, the Child's father is engaged to be married and it appears that he is busy with his new relationships. The family dynamic has historically been that the Child's father is more focused on himself than the Child. h. The child support obligation of the natural father is in arrears and he has been scheduled for contempt hearings fifteen (15) times since the entry of the initial Final Order entered by agreement July 18, 2000. i. The Child has no other natural siblings, but has a step sister, Heather Morrill with whom she is very close. The Child has expressed that she wishes to have the same surname as Heather and Heather supports her request for the change. 6. Petitioner and the Child have resided at 7 Ash Drive, Mechanicsburg, Cumberland County, Pennsylvania 17050 since June 15, 1998. 7. There are no judgments or decrees of like character against the Child. WHEREFORE, Petitioner, Danielle E. Morrill, as Guardian for minor child, Allison E. Clites, respectfully requests the Court, pursuant to the Act of December 16, 1982, P.L. 1309, No. 295, § 2, 54 Pa. Cons. Stat. Ann. § 702, for the following: To schedule a hearing on this Petition; b. To direct that notice be given on this Petition and hearing; and c.
