Rovegno Vice Chairman ANNUAL GUARDIANSHIP REPORT Bazbar~e. erp~ FOR NANCY HOERNER ~ Terry L~ley ~Q Dior June 8, 2009 ~~c~ ~ Report from Guardian of Person ~~-,~ ~ Cumberland County Office of Aging :.:~~~ Janet Paull, Aging Care Manager 3 ~Q~' ~ ~ :. On March 14, 2006 Judge Kevin Hess appointed the Cumberland County C~fice of ~- Aging Emergency Plenary Guardian of Person for Nancy Hoerner and on April 6, 20~° the agency was named Nancy's Permanent Plenary Guardian. On April 18, 2006, following a brief stay in the Behavioral Health Unit of Chambersburg Hospital, Nancy was admitted to the South Mountain Restoration Center, South Mountain, PA where she is currently staying. Nancy was diagnosed with chronic schizophrenia, adjustment disorder with disturbance of conduct, and cognitive disorder which was more recently diagnosed as alzheimer's disease. Nancy has had occasional disagreements with other residents but usually the facility's staff has done a good job in addressing these issues. At one point Nancy was given a private room on the same floor because of continued negative interactions with her room mate. She seems to be much happier with this arrangement. She does have a history of hitting the caregivers for no apparent reason as attention seeking behavior. For social activities she likes to play checkers, visit the cafe and go on the bus trips to a local Sheetz. She occasionally calls her daughter. It is very easy to converse with Nancy but it becomes evident very quickly that she has cognitive problems. On one of my visits, she insisted that her mother lived on the 6th floor of the building where she lives. This is not true. Nancy has been compliant with taking medication. The only medication she is on is aricept 5 mg at bedtime for dementia and tamoxifen 10 mgs 2 times a day for breast cancer prevention since atypical cells were found. It is the request of the Cumberland County Office of Aging that the agency retains Guardianship of Person for Nancy Hoerner. The South Mountain Restoration Center has demonstrated an adequate quality of care for her medical and mental problems. We believe, therefore, that Ms. Hoerner should continue to reside in that nursing facility. ;~ ; r' .. ~, ~'_~ ~..
