ORDER OF COURT
AND NOW, this ~ ~ day of June, 2009, following a hearing, and
finding that Carol A. Delaney is an incapacitated person in need of a plenary guardian of
her person, IT IS ORDERED:
(1) Sara Harris is appointed plenary guardian of the person of Carol A. Delaney.
(2) Sara Harris shall have the authority to decide where Carol A. Delaney shall
live and how care is provided and shall have the authority to authorize and consent to
medical treatment and surgical procedures necessary for the well-being of Carol A.
Delaney.
(3) No bond is required.
(4) The personal guardian shall comply with the reporting requirements set forth
in 20 Pa.C.S. Section 5521(c).
