Petitioner is DENALIA FAYE HEDRICK, who resides at 125 East Shady Lane, Enola, Cumberland County, Pennsylvania 17025. 2. Petitioner has resided at that address since August of 2004. From approximately 1996 to August of 2004, Petitioner resided at 48 Maryland Drive, Carlisle, Cumberland County, Pennsylvania 17013. 3. Petitioner has lived continuously in the same household with AMY MICHELLE TAYLOR since approximately 1998, at the following addresses: 125 East Shady Lane, Enola, PA 17025 From August 2004 to the Present SAIDIS, FTJC1?WI R & LIlVDSAY 26 West High Street Carlisle, PA 48 Maryland Drive, Carlisle, PA 17013 1996 to August 2004 4. Petitioner currently resides at the residence set forth above with her life partner, AMY MICHELLE TAYLOR. 5. Petitioner and her life partner, AMY MICHELLE TAYLOR, have a loving, supported and committed relationship that was celebrated with a union ceremony at the Metropolitan Community Church of the Spirit, in Harrisburg, Pennsylvania, on August 4, 2001. 6. Petitioner desires to assume the name of DENALIA FAYE TAYLOR, in acknowledgement of the close and familial relationship between her and AMY MICHELLE TAYLOR. 7. There are no judgments or decrees of like character of record against Petitioner DENALIA FAYE HEDRICK, and this change of name is not offered in any regard to avoid debts to creditors. WHEREFORE, Petitioner requests that this Court, pursuant to the Act of December 16, 1982, P.L. 1309, No. 295, Section 254 Pa.C.S. §720 to: A. Fix a hearing on this Petition; B.
