AND NOW, comes Adam Brooks Kennedy in and through his mother Amanda Brooke
Doyle and his step-father Daniel Joseph Doyle, by and through their attorney, Andrew H. Shaw,
and files this Petition for Change of Name and in support thereof avers as follows:
1. Adam Brooks Kennedy, (hereinafter "Petitioner") was born on October 28, 1997, and is
currently Eleven (11) years old.
2. Petitioner resides at 312 Walnut Street, Boiling Springs, Cumberland County,
Pennsylvania with his birth mother, Amanda Brooke Doyle, her mother's husband,
Daniel Joseph Doyle and Amanda and Daniel Doyle's biological children, Cohen Lucas
Doyle and Avery O'Reilley Doyle.
3. Petitioner's biological father is Michael Epperson, who is in current military service at
Fort Polk, Louisiana.
4. Petitioner seeks to have his name changed from Adam Brooks Kennedy to Adam Brooks
Doyle.

Petitioner seeks a change of name because he desires to share the same last name as his
birth mother, birth mother's husband and his birth mother's and birth mother's husband's
two children.
Petitioner currently has the last name of his mother's maiden name, and does not have his
biological father's last name.
Petitioner's biological father, Michael Epperson, serves in the Armed Services and
maintains telephone contact with Petitioner approximately once every two weeks and
personal contact approximately one to two times per year. Petitioner's biological father
provides financial support to Petitioner through child support.
Petitioner's residence for the past 5 years has been as follows:
a. 312 Walnut Street, Boiling Springs, Cumberland County, Pennsylvania (September 2004
to present);
b. 729 West Louther Street, Carlisle, Cumberland County, Pennsylvania (February 2004 to
September 2004);
c. 1402 Viewmore Drive, Carlisle, Cumberland County, Pennsylvania (October 1997 to
February 2004).
WHEREFORE, Petitioner respectfully requests this Honorable Court to grant his request
to change his name from Adam Brooks Kennedy to Adam Brooks Doyle.
