by the Court without further notice for any money claimed in complaint or for any other claim or
relief requested by the plaintiff. You may lose money or property or other rights important to
you.

COMPLAINT
The plaintiff, Cumberland Valley Rental, is a business located at 1 I 1 West King Street,
Shippensburg, Cumberland County, Pennsylvania 17257.
1 The defendant, Taylor Home Remodeling, Inc., is a corporation with its principal place of
business located at 5103 Carlisle Pike, Mechanicsburg, Cumberland County, Pennsylvania
17055.
3. On January 29, 1999 the parties entered into a written contract with an effective date of
January 29, 1999, for the rental and cleaning of uniforms for the employees of the
defendant, Taylor Home Remodeling, Inc., in Mechanicsburg, Pennsylvania. A copy of
the contract is attached hereto as Exhibit A.
4. The defendant, Taylor Home Remodeling, Inc., breached the contract by refusing to
submit payment for services provided. A copy of the last statement submitted to the
defendant is attached hereto as Exhibit B.
5. As a result of the breach of contract by the defendant, Taylor Home Remodeling, Inc., the
plaintiff, Cumberland Valley Rental, has sustained a loss for total damages in the amount
of $9,463.10, plus costs.
WHEREFORE, the plaintiff claims of the defendant the sum of $9, 463. 10, plus costs, an
amount requiring compulsory referral to arbitration under the local rules of court.

from the Company. Customer acknowlgdges that the Company will he obligated to wake a substantial investment in uniforms andlor equipment to
fulfill this Agreement, accordingly, the tuts of this Agreement shall be for a period of it months, frog and after date hereof. Either party
lay cancel by giving written notice to the other party at least 90 days prior to the expiration date, unless so terminated this contract shall
be automatically extended for like ten. In the event that during the ten of this Agreement for any renewal termj Customer has any objections
or complaints about the services or service charges being charged, Customer shall immediately notify Company in writing of its objections or
complaints and Company shall have a reasonable time after delivery of such notice In which to take such corrective action as say be necessary.
The fellers of Customer to notify Company to writing of any objection or complaint within 10 days after the occurrence of the facts giving rise
to any such objection or complaint shall constitute a waiver of such objection or complaint by Customer and shall not thereafter he the basis
for termination, offset, or counterclaim.

PRICE 6. All prices are guaranteed for a period of 2 years from the date of Installation. Cumberland Valley Rental reserves the option to adjust
CHARGES all rental prices every 2 years, not to exceed the increase in the Consumer Price Index for the previous 2 year period, on each anniversary date
of this Agreement. No price shall exceed 11.
TERMINATE 7. If the Customer fails to comply with this Agreement or if Customer elects to terminate it for any reason prior to the expiration of the tens
above-stated, the Customer will pay the Company as liquidation charges, in amount equal to forty pgtj percent of the total regular weekly rental
multiplied by the number of weeks remaining in the ten, plus the current replacement value of any garment not returned to the Company. In
enforcing any tens of this agreement, the customer shall pay any costs including reasonable attorney fees or arbitration fees incurred by
company.
B. This Agreement shall be binding upon and shall ensure to the benefit of the personal representatives, successors, and assigns of the
respective parties hereto,
9. This Agreement is entire and includes all understandings of the parties. No waivers or statements made by any representative of the Company
shall be valid unless set forth hereto.
