DECREE
AND NOW, this / y day of qwh , 2009, upon review of
the attached Petition for Appointment of Guardian and there being no response to the Rule issued upon
Northwestern Mutual Life Benefits Division, it is hereby ordered that John Borkenhagen and Diana
Borkenhagen are appointed guardians of Hannah Borkenhagen for the purposes of receiving and
holding in trust funds currently held by Northwestern Mutual Life Benefits Division in favor of their
daughter Hannah Borkenhagen. It is further ordered that upon receipt of this Order, Northwestern
Mutual shall release said funds to John Borkenhagen and Diana Borkenhagen. Said funds are to be
deposited into Edward Jones Custodial Mutual Fund Account Number 242-10417. Said funds may be
withdrawn by Hannah Borkenhagen when she attains the age of majority. Prior to Hannah
Borkenhagen attaining the age of majority, said funds shall be withdrawn only by further order of
court.
