Crass Secretary Terry L Barley Director ~~ r n _, ~~ G © c.._ - ~~~ c r= a~ ~ r:~ ~. :L7 PJ C!~ 1,.~ -; ~ ~ :~ ` On June 20, 2005, Permanent Guardianship of Person and Estate for Theresa Ceja was granted by Judge Kevin Hess to Cumberland County Office of Aging. Prior to this appointment Mrs. Ceja resided in her own apartment at 1108 Yverdon Drive Apt. C4 Camp Hill, Pennsylvania. She was admitted to the Forest Park Health Center Carlisle on May 27, 2005 after the outcome of an Emergency Guardianship hearing. Mrs. Ceja continues to reside at Forest Park Health Center. She was moved from the dementia unit to the nursing unit in August 2007. Due to her severe cognitive loss she was no longer benefiting from the activities and structure of the dementia unit. Her medical conditions have declined but she has been able to maintain. She continues to be treated for senile dementia Alzheimer's type, depression, Parkinson's, PVD, osteoporosis, hypertension and anxiety. Her cognitive status has also declined and she is now non verbal. The staff at Forest Park Health Center provides 24 hour supervision, maximum hands on assistance with activities of daily living, two assists for all transfers, monitoring of skin as she is prone for breakdown and providing necessary verbal cues and redirection. Mrs. Ceja sees her doctor on a routine basis. Mrs. Ceja no longer sees the psychiatrist as she has been emotionally stable and has not displayed behavioral problems. The psychiatrist discontinued her taking Depakote. Mrs. Ceja has suffered a physical decline and is non-ambulatory. She has been using a Geri chair which has allowed her to be a more active passive participant with the different activities. Her participation rate is 90% and she enjoys watching others. She enjoys the activities in the afternoon which include Bible study, Sing Along, Bingo and cooking. Mrs. Ceja physically is unable to participate but thoroughly enjoys participating as an observer. Last year the facility had an ice cream social and Mrs. Ceja enjoyed a strawberry sundae. Mrs. Ceja is no longer on a regular diet but is on a pureed diet with thin liquids. Mrs. Ceja was evaluated in March by a speech therapist L:M.1I1, US A'I' aging@ccpa.net OK VISI"f OUK WEBSII'L AT www.ccpa.net/aging due to her pocketing food. It was determined at that time that Mrs. Ceja no longer had a gag reflux. She eats approximately 75% of her meals most of the time and has maintained her weight. Mrs. Ceja due to her significant decline in heath was placed on Hospice on March 3rd of this year. Aging made this decision as we believed it would be in Mrs. Ceja's best interest as Hospice focuses on the quality of life and comfort care. Celtic is providing hospice services that are focusing on the physical, emotional and spiritual needs of Mrs. Ceja. Hospice staff tends to her needs several times a week. Mrs. Ceja sleeps a lot but is holding her own. Funeral arrangements were made with Hollinger Funeral Home in Mt. Holly. Mrs. Ceja has an irrevocable burial trust with Sovereign bank. Client is visited every 30-60 days by the care management staff. In addition to visiting client, care plans are attended every 90 days and the medical record is reviewed and discussed with the nursing staff. The staff at the nursing home provides updates and notifications of any changes that Theresa may have experience. It is the request of the Cumberland County Office of Aging that the Agency retains Guardianship of Person and Estate of Theresa Ceja. Forest Park Health Center has demonstrated an adequate quality of care for client by managing her daily needs and continuously monitoring her medical conditions. Mrs. Ceja's appearance is always good and she appears to be content in her present living situation. Therefore, we believe that Mrs. Ceja should continue to reside at Forest Park Health Center.
