PETITION FOR NAME CHANGE.
AND NOW, comes William M. Zeladonis, by and through his Attorney, Jane Adams,
Esquire, and respectfully requests this Honorable Court to change his name for the following
reasons:
1. Petitioner, William M. Zeladonis, (hereinafter referred to as "Petitioner") currently
lives at 19 Abbey Court, Carlisle, Cumberland County, Pennsylvania, 17013.
2. Petitioner William M. Zeladonis, is ninety (90) years old. His date of birth is June 4,
1919. (See copy of birth certificate, attached hereto as Exhibit A).
3. Since his youth, William M. Zeladonis has informally gone by the name of Vincent
Zeladonis. This name is reflected on his certificate of birth and baptism, issued by his church, a
copy of which is attached hereto. (See copy of birth and baptism, attached hereto as Exhibit B).
4. Such name has been used consistently since his youth and not for the purpose of fraud.
5. Petitioner wishes to change his name formally to Vincent M. Zeladonis.
6. Petitioner believes that changing his name formally would avoid confusion regarding
medical and other records.
7. Petitioner has no outstanding credit debt that could be avoided through a name change.
8. Petitioner has no outstanding federal or state tax debt that could be avoided through a
name change.
WHEREFORE, Petitioner requests that this Honorable Court Order change his name.
