The Petition of Dennis T. Owens and Amanda Martin Owens to be appointed as
Guazdians for their minor daughter, Madeline Rose Owens, respectfully represents that:

Petitioners are 47 and 34 yeazs of age, respectively, and reside at 508 Partridge Court,
Hampden Township (Mechanicsburg), Cumberland County, Pennsylvania, 17050.
Petitioners aze husband and wife, and aze the natural pazents of Madeline Rose Owens, a
minor, age 2, born on October 6, 2006. .
Petitioner, Dennis T. Owens, is a surviving brother of Michael Thomas Owens, who died
on May ICJ , 2008, a resident of Philadelphia County, Pennsylvania.
The Decedent, Michael Thomas Owens, did not have a Will.
The minor, Madeline Rose Owens, is the named beneficiary of life insurance proceeds
from The Hartford Life & Accident Insurance Company, Policy No. ADD-10900, in the
amount of Fifty Thousand Dollars ($50,000.00). The minor has no other assets.
In order to receive the proceeds of the aforementioned life insurance policy, The Hartford
Life & Accident Insurance Company requires that Guardians be appointed for the minor.
The proposed Guardians have no interest adverse to the minor.
8. Petitioners have individually executed Consents to Serve as Guazdian of the Estate,
which Consents are attached to this Petition, and they further consent to the appointment
of a Co-Guardian, as required by 20 Pa.C.S.A. §5112(3), in the person of the
undersigned.
WHEREFORE, Petitioners respectfully request that this Honorable Court appoint Dennis
T. Owens and Amanda Martin Owens as Guazdians of the Estate of Madeline Rose Owens, a
minor.
