AND NOW, comes the Petitioner, Kristin Davies-Crone, by and through her counsel, Timothy J. Colgan, of Colgan Marzzacco, LLC, and files the within Petition for Change of Name pursuant to 54 Pa.C.S.A. §§ 701 et seq., and in support thereof, avers as follows: 1. Petitioner is Kristin Davies-Crone, an adult individual who resides at 606 Grandview Avenue, Camp Hill, Cumberland County, Pennsylvania 17011. 2. Petitioner is seeking to change her surname. 3. The Petitioner wishes to change her name to add the maiden name of her Mother to her surname. 4. Petitioner wishes to formally change her name from Kristin Davies-Crone to Kristin Davies-Croft. 5. Petitioner's residence is 606 Grandview Avenue, Camp Hill, Cumberland County, Pennsylvania. 6. During the past five years, the Petitioner has resided at the following addresses: Dates Address A. August 15, 2009 to 606 Grandview Avenue Present Camp Hill, PA B. October 9, 2006 to 2445 Heather Road August 15, 2009 York, PA 17408 C. August 2002 to 577 Steinhour Road October 9, 2006 York Haven, PA 17370 7. There are no outstanding judgments, liens or decrees of record against the Petitioner. 8. There are no minor children who would be affected by the requested name change.
