PETITION TO CHANGE NAME
PURSUANT TO 54 Pa.C.S.A. 701 et. sec.
AND NOW, comes Wayne Melnick, Esquire, on behalf of the Petitioner, William
Allen Fagan, Jr., who respectfully requests this Court allow him to change his name to,
William Allen, and in support thereof avers as follows:
1. Petitioner is William Allen Fagan, Jr., an adult individual with a date-of-birth
of April 23, 1968, who resides at 1034 Northfield Drive, Carlisle, Cumberland County,
Pennsylvania.
2. At the time of his birth, Petitioner was given the name William Allen Fagan,
Jr.,.
3. Petitioner desires to change his name to William Allen.
5. Petitioner desires to change his name to William Allen due to a dislike of his
last name and its association with his father. Petitioner seeks to end that paternal association
inherent with the sharing of a last name and the junior suffix.
6. Over the past five years petitioner has resided in the following locations:
1034 Northfield Drive, Carlisle, Cumberland County, Pennsylvania; and
8000 Christopher Wren Drive, Wexford, PA, 15090.
7. There are no judgments or decrees of record against petitioner.
8. Attached to this Petition is a fingerprint card containing the fingerprints of
the Petitioner. The Petitioner's fingerprints were placed on the fingerprint card on July 8,
2009, at the Carlisle Police Station, in Carlisle. Said fingerprints are attached for the purpose
of forwarding the same to the Pennsylvania State Police as required by 54 Pa.C.S. §702(b)
WHEREFORE, petitioner respectfully requests this Honorable Court to enter an
Order granting the petitioner's request to change his name from William Allen Fagan, Jr., to
William Allen.

AND NOW, this 1St day of September, 2009, upon consideration of the attached
Petition To Change Name Pursuant to Pa. C.S.A. §701 et. seq., a hearing is scheduled for
Thursday, October 22, 2009, at 2:00 p.m., in Courtroom No. 1, Cumberland County
Courthouse, Carlisle, Pennsylvania.
IT IS FURTHER ORDERED that notice of the filing of said Petition and the
aforesaid date, time and place of the hearing be published once in the Cumberland Law
Journal and a newspaper of general circulation in the County at least 10 days prior to the
hearing; in addition, Petitioner shall comply with all other requirements of C.C.R.P. 5,
and the Act of December 16, 1982, P.L. 1309, §6 (see 54 Pa. C.S. §701 [Historical and
Statutory Notes] [Main Volume]), and 54 Pa. C.S. §§701 et seq.

Police, upon receipt of this notice, shall include the change of name information in the Central Repository as
provided for in 18 PA C.S., Chapter 91 (relating to criminal history record information).
