Finnen, your Petitioner, files this Petition for Settlement of a Small Estate under the provisions of Section 3102 of the Probate, Estates and Fiduciaries Code and in support thereof avers that: (1) Your Petitioner, Sarah W. Finnen, is a competent adult residing at 812 Gobin Street, Carlisle, Pennsylvania 17013, and is the daughter and sole heir of the above decedent. (2) Florence H. Pittenger, also known as Florence M. Woods, mother of the Petitioner, died on December 27, 2006 at the age of 98 years, but prior thereto lived and was domiciled at Claremont Nursing Home, 1000 Claremont Road, Carlisle, Pennsylvania, Cumberland County, Pennsylvania. She died without a Will and no Letters of Administration have been issued. A copy of decedent's Death Certificate is attached hereto as Exhibit "A." (3) Florence H. Pittenger had no probate estate when he died other than the following: 177 Shares of MetLife, Inc. Stock valued at approximately $4,632.03. MetLife, Inc. statement and stock quote attached hereto as Exhibit "B." Outstanding check from MetLife, Inc. dated November 6, 2007 in the amount of $86.58. A copy of the uncashed check is attached hereto as Exhibit "C." Outstanding dividends and Miscellaneous Stock with MetLife, Inc. held as unclaimed property with the Bureau of Unclaimed Property, PO Box 1837, Harrisburg, PA 17105-1837 in the amount of $1,829.23. Affidavit and Indemnification Agreement from the Bureau of Unclaimed Property listing property description is attached hereto as Exhibit "D." (4) Your Petitioner avers that there are no creditors of the decedent and no claims unpaid known to your Petitioner. WHEREFORE, your Petitioner respectfully requests that an Order be made authorizing Sara W. Finnen transfer and liquidate all shares with MetLife, Inc., request and cash replacement check from MetLife, Inc. in the amount of $86.58, and to claim all unclaimed property with the Bureau of Unclaimed Property, PO Box 1837, Harrisburg, PA 17105-1837, pursuant to Section 3102 of the Probate, Estates and Fiduciaries Code.
