Township of Hampden ("Claimant") files this Municipal Claim against Steven A.
Sheely, Jr. and Steven A. Sheely, Jr. ("Owners") and all that certain tract of land including
improvements thereon, owned by the Owners and described as follows:

for unpaid sewer and/or trash services rendered from October 1, 2008 to August 1, 2009. Said
services were rendered and charged pursuant to Hampden Township Code of Ordinances
Chapter 18, Part 2 and/or Chapter 20, Part 1, as amended.
Attached hereto is a statement of the kind and character of the services so rendered and
the prices charged therefor, said statement being a true and correct copy of the Owners' account
with the Claimant, which sum of $466.25 was duly assessed against the above Owners and
premises beginning October 1, 2008 and for which sum, which includes interest, costs and
penalties, a lien is claimed against the above premises in accordance with the Municipality
Authorities Act of June 19, 2001, P.L. 287, 53 P. S. § 5601, et sect., as supplemented and
amended and the Municipal Claims and Tax Liens Act of May 16, 1923, P. L. 207, 53 P. S.
