Sheely, Jr. ("Owners") and all that certain tract of land including improvements thereon, owned by the Owners and described as follows: 6201 Haymarket Way Mechanicsburg, PA 17050 Tax Property Map: 10-19-1606 Parcel No.: 120 for unpaid sewer and/or trash services rendered from October 1, 2008 to August 1, 2009. Said services were rendered and charged pursuant to Hampden Township Code of Ordinances Chapter 18, Part 2 and/or Chapter 20, Part 1, as amended. Attached hereto is a statement of the kind and character of the services so rendered and the prices charged therefor, said statement being a true and correct copy of the Owners' account with the Claimant, which sum of $466.25 was duly assessed against the above Owners and premises beginning October 1, 2008 and for which sum, which includes interest, costs and penalties, a lien is claimed against the above premises in accordance with the Municipality Authorities Act of June 19, 2001, P.L. 287, 53 P. S. § 5601, et sect., as supplemented and amended and the Municipal Claims and Tax Liens Act of May 16, 1923, P. L. 207, 53 P. S.
