FINAL DECREE
AND NOW, this 3'~ day of ~^" ,'2009, upon consideration of the
Petition for Adjudication of Incapacity and Appointment of a Guardian over the Person and
Estate of Theodore Syphax, and after a hearing on the matter, this Court hereby ORDERS AND
DECREES that: ~
1. Theodore Syphax is adjudged an incapacitated person and Penns

Guazdianship Association is appointed guazdian of his Person and Estate. The th
Permanent Guazdian of the Person and Estate shall be performed in accordance heT„u

provisions of 20 Pa.C.S.A. § 5501 et seq. and as limited therein. ~
a. Pennsylvania Guazdianship Association, the Guazdian of the Person and
Estate, shall file a Report on the social, medical and other relevant conditions at
least once during the first yeaz of appointment and annually thereafter as required
by 20 Pa. C.S.A. § 5521(c).
b. Pennsylvania Guazdianship Association, the Guardian of the Person and
Estate, shall have the authority to decide where Theodore Syphax shall live and
how meals, personal caze, transportation and recreation shall be provided.
Pennsylvania Guazdianship Association shall also have the authority to authorize
and consent to medical treatment and surgical procedures necessary for the well-
being of Theodore Syphax.

c• Within sixty (60) days of the death of Theodore Syphax or an adjudication
of capacity, Pennsylvania Guardianship Association shall file a Final Report with
the Court pursuant to 20 Pa. C.S.A. § 5521(c)(2).
d. In the event Theodore Syphax has a safe deposit box, an Official
Examiner of this Court shall be in attendance when said safe deposit box is
opened. The Certificate of the Official Examiner and her examination of the
assets in such safe deposit box shall be submitted to the Court, and, when
approved by the hearing judge, shall be filed with the record in this case. The
amount and manner of compensation for these services of the Official Examiner
shall be determined by the hearing judge.
3. Pennsylvania Guardianship Association shall serve upon and read to Theodore
Syphax this Decree, and advise him of his right to file exceptions to this Decree within twenty
(20) days of the date herein, of his right to appeal to the Superior Court within thirty (30) days of
the denial of the exceptions, and of his right to petition the Court at any time to modify or
terminate the plenary guardianship if there is a significant change in his capacity or if his plenary
guardian fails to perform its duties in accordance with the Court's Decree.
4. Pennsylvania Guardianship Association shall be entitled to receive compensation
for its services as guardian of the Person or the Estate of Theodore Syphax, an incapacitated
person. This compensation shall be paid monthly, beginning with the month of appointment.
The amount of such compensation shall be in accordance with the current fee schedule of
Pennsylvania Guardianship Association that is in effect when services are performed; provided,
however, if Theodore Syphax is now or becomes eligible for Medical Assistance, the fee amount
shall be no less than the maximum monthly amount allowable under the policy of the

Medical Assistance, this amount is currently $100.(
