4. The mother of the child is Amanda Riccio residing at 202 East Portland Street
Mechanicsburg. She is not married.
5. The father of the child is Matthew T. Baum, residing at 121 East Simpson Street,
Mechanicsburg, Cumberland County, PA, 17055. He is not married.
6. The relationship of Plaintiffto the child is that of father. The Plaintiff currently
resides with the following person(s):

7. The relationship of the defendant to the child is that of mother. The Defendant

8. Plaintiff has not participated as a party or witness, or in another capacity, in other
litigation concerning the custody of the child in this or another court. Plaintiff has no
information of a custody proceeding concerning the child pending in a court of this
Commonwealth. Plaintiff does not know of a person not a party to the proceedings who has
physical custody of the child or claims to have custody or visitation rights with respect to the
child.
9. The best interest and permanent welfare of the child will be served by granting shared
legal custody of the child to the both parties and partial physical custody to the Plaintiff. In this
manner the child may be enriched by healthy relationships with each parent.
10. Each parent whose parental rights to the child has not been terminated and the person
who has physical custody of the child have been named as parties to this action. All other
persons, names below, who are known to have or claim a right to custody or visitation of the
child will be given notice of the pendency of this action and the right to intervene:

at 39 West Main Street, Meehaniesburg, PA 17055 on the 6th day of February ,200l, at 1:00 p.m.
for a Pre-Hearing Custody Conference. At such conference, an effort will be made to resolve the issues in dispute; or
if this cannot be accomplished, to define and narrow the issues to be heard by the court, and to enter into a temporary
order. All children age five or older may also be present at the conference. Failure to appear at the conference may
provide grounds for entry of a temporary or permanent order.

consideratic~ of the attached Custody Conciliatzon Report,
and directed as follows:

it is ordered
1. The Father, Matthew Baum, and the Mother, Amanda Riccio, shall
have shared legal custody of Cainan Riccio-Baum, born September 9, 1998.
Each parent shall have an equal right, to be exercised jointly with the
other parent, to make all major non-emergency decisions affecting the
Child's general well-being including, but not limited to, all decisions
regarding his health, education and religion.
2. The Father shall have a minimum of 4 periods of supervised
visitation with the Child at Parent Works or other supervising agency
selected by agreement of the parties. The parties shall cooperate in
scheduling the first session of supervised visitation as soon as possible.
3. The Father shall continue in his course of counseling with Michael
Kline until such t/me as his release by recommendation of the counselor.
The Mother shall follow the reco~aendations of the counselor with respect
to her participation in joint counseling with the Father. The Father shall
sign any releases or authorizations necessary for the Mother or her counsel
to obtain information from the Father's counselor with respect to the
Father's attendance at and progress in the counseling sessions.
4. After the Father has completed the supervised periods of
visitation provided in this Order, if the parties are not able to reach an
agreement as to ongoing custody arrangements, counsel for either party may
contact the Conciliator to schedule an additional Custody Conciliation
Conference.
