The following, along with the enclosures, is the report regarding the estate and pers$~ of Chseles .
C. Awkerman, per our appointment as guardians. ~ rv
As of August 2008, Charles has resided in the Alzheimer's Wing of the Sarah Todd Memorial
Home at 1000 W. South St., Carlisle, PA, 17013. Since the time of the guardianship hearing in
July of 2008, his mental capacity has deteriorated further. He is not able to communicate, he falls
frequently, and he is incontinent. We believe he still recognizes our two sisters and us, but he
does not recognize his grandchildren or any of his other life-long friends. He is receiving
excellent care in his current setting and the staff strives to provide for all of his needs. His overall
state of health has otherwise not changed.
Between July 29, 2008 (date of the court appointed guardianship) and 6/30/09, Paula visited him
22 times and spent a total of 37 hours and 45 minutes with Charles. In the same time frame,
Linda visited him 101 times and spent over 200 hours with him. We have spent time with him on
his birthday and on all holidays. In addition to time spent visiting in the nursing home, one or
both of us continue to provide for his needs in the following ways:
• Washing and ironing all of his personal laundry. Mending as needed.
• Providing his clothing and other personal items as needed.
• Cutting his fingernails regularly (he will not allow staffto do this)
• Initially, taking him to dentist and specialist appointments. He is no longer able to leave the
home, so one of us must go to the home to assist with all of his specialty care appointments -
dentist, podiatrist, etc. (he will not cooperate and allow care to be provided unless one of us
is there with him).
• Taking him to the in-home hairdresser for haircuts monthly and taking him weekly to get his
hair washed (he will not allow staff to do this when they shower him).
• Initially, we took him to family gatherings on holidays (Labor Day and Thanksgiving), but
we have not been able to take him out since November of 2008 because of concerns for being
able to provide for his safety in an uncontrolled environment.
• We have taken his 18 month old great-granddaughter to visit him several times, when she is
here from Lansdale.
• Attended quarterly care plan conferences.
• Taken over 50 phone calls from nursing home staff, who were reporting various issues,
concerns, and problems. Each of these calls involves discussing Charles's care and treatment
with nursing home staff.
• Spoken with Dr. Dell (his primary care provider) periodically to find out his assessment of
Charles's condition, to request needed physician orders, and to provide information regarding
Charles's condition that we have become aware of.
• Handle all of his financial matters, including paying bills. We have recently spent many
hours dealing with Bank of New York attempting to have my deceased mother's name
removed from the stock holdings he has.

As is evident from the list of needs that we fulfill, we feel that this guardianship should continue
as ordered. We have fulfilled Charles's needs both willingly and lovingly for many years and
have the full support of our other two sisters, Pamela B. Burns and Emily J. Awkerman. We
communicate with each other and with Pamela and Emily frequently.
