AND NOW, this ~-Z"' day of September, 2009, upon consideration of the
within petition requesting the award of an estate of Elizabeth M. Halford, a minor, and
Amanda R. Halford, a minor, without the appointment of a guardian or the entry of
security, and pursuant to § 5101 of the Probate, Estates and Fiduciaries Code, 20
Pa.C.S. § 5101, and also in consideration of local rule C.C.O.C.R. 12.5-3, IT IS
ORDERED that the Lincoln Financial Group may pay the proceeds of Annuity Contract
No. 95-406779 for the benefit of the respective minor, to the New Cumberland Federal
Credit Union for Elizabeth M. Halford, and to the New Cumberland Federal Credit Union
for Amanda R. Halford. The proceeds for each minor shall be deposited into federally
insured accounts; one in the name of Elizabeth M. Halford, born August 6, 1998, and
the other in the name of Amanda R. Halford, born October 18, 2001. Each account
shall contain the following notation: "NO WITHDRAWAL CAN BE MADE PRIOR TO
THE ENTRY OF THE MINOR OBTAINING HER MAJORITY EXCEPT BY AN ORDER
OF A COURT OF COMPETENT JURISDICTION."
Counsel, David J. Lenox, Esquire, shall file with the Prothonotary, and forward a
copy to this chambers, proof of compliance with this order.
