PETITION FOR CHANGE OF NAME
1. Your Petitioner, Rachel Joy Rhine, has a home address of 229 Graham Street,
Carlisle, Cumberland County, Pennsylvania and desires to change her name to
Rachel Joy Kulikauskas Rhine.
2. Petitioner desires to change her name for the following reasons:
a. To use the family name of her husband as it was originally spelled before
it was changed by family members several years ago from Kulikauskas to
Kulikosky.
b. There are no judgments or decrees of like character of record against your
Petitioner.
c. Petitioner wishes to have her name end as Kulikauskas Rhine since she
has a daughter from a previous relationship named Alexis Rhine.
d. Petitioner is not requesting this name change for any fraudulent or illegal
purposes.
WHEREFORE, Petitioner prays that Your Honorable Court enter an Order changing
Petitioner's name from Rachel Joy Rhine to Rachel Joy Kulikauskas Rhine.
