PETITION FOR CHANGE OF NAME
1. Your Petitioner, Michael Albert Kulikosky, has a home address of 229
Graham Street, Carlisle, Cumberland County, Pennsylvania and desires to
change his name to Michael Albert Rhine Kulikauskas.
2. Petitioner desires to change his name for the following reasons:
a. To use the family name as it was originally spelled before it was changed
from Kulikauskas to Kulikosky several years ago by family members.
b. There are no judgments or decrees of like character of record against your
Petitioner.
c. Petitioner wishes to add the last name of his wife, Rachel Joy Rhine, as
part of his new name.
d. Petitioner is not requesting this name change for any fraudulent or illegal
purposes.
WHEREFORE, Petitioner prays that Your Honorable Court enter an Order changing
Petitioner's name from Michael Albert Kulikosky to Michael Albert Rhine Kulikauskas.
