PETITION FOR NAME CHANGE
1. Petitioner is Jennifer Louise Cruver, an adult female who resides at 125 Kerrs'
Road, Carlisle, Cumberland County, Pennsylvania 17015 (hereinafter,the^?P-etitioner' or
"Jennifer")
2. Jennifer was born on February 28, 1981 with the name of Jennifer Louise ,
Cruver.
3. Jennifer is married to Joshua Thaddeus-Morse Cruver, with a date of
marriage of August 14, 2009. Upon marriage, Joshua adopted Jennifer's last name as
his last name, and began using the middle name of Thaddeus-Morse to maintain his
last name.
4. The parties wish to incorporate both Joshua's mother's maiden name and
Jennifer's maiden name into their surname.
5. Joshua's mother's maiden name was Kibbe, an original spelling of which is
Attorney for Petitioner
Kibi. By "marrying" Jennifer's surname and Joshua's mother's maiden name, the
parties wish to honor and respect their family members in their chosen surname. As
such, the parties wish to use the surname CruverKibi.
6. Therefore, Jennifer desires to change her name from Jennifer Louise Cruver
to Jennifer Louise CruverKibi.
7. Joshua has filed a separate petition to change his name from Joshua
Thaddeus-Morse Cruver to Joshua Thaddeus-Morse CruverKibi.
8. There are no judgments or decrees of record or any other matter of like
character against Petitioner.
9. Jennifer does not have a criminal background-,

In accordance with the Name Change Act of 1998, the fingerprint cards of the above named individual, who
has applied for a name change, has been searched in the files of the Pennsylvania State Police, Criminal Records and
Identification Division Central Repository, and:
(x) This person is not subject to 18 PA C.S. Chapter 91, and the fingerprints have been destroyed.
( ) This person is subject to 18 PA C.S. Chapter 91, and the name change has been noted on the person's
criminal history record information.
( ) A criminal record was revealed containing felony convictions. IN ACCORDANCE WITH THE NAME
CHANGE ACT, THE COURT SHALL NOTIFY THE PENNSYLVANIA STATE POLICE WHEN A NAME
CHANGE FOR A PERSON CONVICTED OF A FELONY HAS BEEN ORDERED. The Pennsylvania State
Police, upon receipt of this notice, shall include the change of name information in the Central Repository as
provided for in 18 PA C.S., Chapter 91 (relating to criminal history record information).
