PETITION FOR NAME CHANGE
1. Petitioner is Joshua Thaddeus-Morse Cruver, an adult male who resides. at
125 Kerrs Road, Carlisle, Cumberland County, Pennsylvania (7015 (h?ereinaftet, the
"Petitioner" or "Joshua")
2. Joshua was born on November 12, 1977, with the nameof Joshua Thaddeus
Morse.
3. Joshua is married to Jennifer Louise Cruver, with a date of marriage of
August 14, 2009. Upon marriage, Joshua adopted Jennifer's last name. as his last
name, and began using the middle name of Thaddeus-Morse to maintain his last name.
4. The parties wish to incorporate both Joshua's mother's maiden name and
Jennifer's maiden name into their surname.
5. Joshua's mother's maiden name was Kibbe, an original spelling of which is
Kibi. By "marrying" Jennifer's surname and Joshua's mother's maiden name, the
parties wish to honor and respect their family members in their chosen surname. As
such, the parties wish to use the surname CruverKibi.
6. Therefore, Joshua desires to change his name from Joshua Thaddeus-Morse
Cruver to Joshua Thaddeus-Morse CruverKibi.
7. There are no judgments or decrees of record or any other matter of like
character against Petitioner.
8. Joshua does not have a criminal background.

has applied for a name change, has been searched in the files of the Pennsylvania State Police, Criminal Records and
Identification Division Central Repository, and:

CHANGE ACT, THE COURT SHALL NOTIFY THE PENNSYLVANIA STATE POLICE WHEN A NAME
CHANGE FOR A PERSON CONVICTED OF A FELONY HAS BEEN ORDERED. The Pennsylvania State
Police, upon receipt of this notice, shall include the change of name information in the Central Repository as
provided for in 18 PA C.S., Chapter 91 (relating to criminal history record information).
