I am the Plaintiff in the above action and because of my current financial situation I am unable to pay fees and costs for filing of Complaint of Divorce. 2. My responses to the questions below relating to my ability to pay the fees and costs of proceeding with filing are true and correct. A: Are YourPresently Employeed? No 1. Long , who currently resides at 310 25th St. (address) Altoona (city) Blair (county) PA (state) since Feb. 1 2009(date), but is currently incarcerated in Cumberland County Prison at 1101 Claremont Rd, Carlisle, PA 17013. 2. Defendant is Lisa M. Long , who currently resides at 3B S. Deleware Ave. (address) Glens falls (city) Warren (county) New York (state) Since May 1,2009 (date). 3. Edwirrd J. Long (plaintiff) has been a bona fide resident in h. the Commonwealth for at least six months prior to filing this Complaint. 4. The plaintiff and defendant were married on June 18, 2008 (date) at Albany (city) Newv York/Albany (state/county). 5. There have been no prior actions of divorce or annulment between the parties except October 13th 2009 . 6. The Marriage is irretrievably broken. 7. Plaintiff has been advised that Counseling is available and that plaintiff may have the right to request that the court require the parties to participate in counseling. 8. Plaintiff requests the court to enter a Decree of Divorce. I verify that the statements made in this Complaint are true and correct. I understand that false statements made herein are subject to penalties under 18 Pa.C.S. §4904, relating to unsworn falsification to authorities.
