Pursuant to Pa.C.S. 240 & 1920.62, Edward J. Long
Plaintiff above named, here by applies for leave to proceed IN
FORMA PAUPERIS on the ground that he is unable to pay the fees
and costs of filing the Complaint of Divorce as set forth in
the attached Verified Statement.

Edward J. Long , states under penalties of 18 Pa.C.S. §4904,
unsworn falsification that:
1. I am the Plaintiff in the above action and because of my
current financial situation I am unable to pay fees and costs
for filing of Complaint of Divorce.
2. My responses to the questions below relating to my ability
to pay the fees and costs of proceeding with filing are true and
correct.
A: Are YourPresently Employeed? No
1. If the answer is yes, please state the amount
of your salary and wages per month, and give the name of your
employer: Not Employeed
2. If the answer is NO, please state the date of ;7o
your last employment and the amount of salary and wages per month
you reeived: June,3092009? and wages were $400.00/Month.
B. Have you received within the last twelve months
any income from a business, profession, self employment, or in th
form of rent payments, interest, dvidens, pensions, annuities,
social security benefits, support payments, public assistance,
or any other form of source? : YES
1. If the answer is YES, describe each source of
income received: I was self employeed as a Tattoo Artist for
the last year and a half, making a roximatly $400.00 per month.
C. Do you own any checking and/or savings accounts?:

D. Do you own any real estate, stocks, bonds, notes
automobiles, or other valuable property (excluding ordinary home
furnishings, and clothing)?:
NO
E. List the person(s), if any , whom are dependant on
you, and state the relationship to that/those person(s): Jazmyn
M. R. Long, age 7, daughter, Dakota E Long, age 4, Son and
Edward J. Long Jr., Age 10 Months, Son.
F. Please state any outstanddng debst and/or
obligationb: NONE AT THISnTIME

COMPLAINT UNDER SECTION 3301(c) OR
3301(d) OF THE DIVORCE CODE
1. Plaintiff is Edward J. Long , who currently resides at 310
25th St. (address) Altoona (city) Blair (county) PA (state)
since Feb. 1 2009(date), but is currently incarcerated in
Cumberland County Prison at 1101 Claremont Rd, Carlisle, PA
17013.
2. Defendant is Lisa M. Long , who currently resides at 3B
S. Deleware Ave. (address) Glens falls (city) Warren (county)
New York (state) Since May 1,2009 (date).
3. Edwirrd J. Long (plaintiff) has been a bona fide resident in h.
the Commonwealth for at least six months prior to filing this
Complaint.
4. The plaintiff and defendant were married on June 18, 2008
(date) at Albany (city) Newv York/Albany (state/county).
5. There have been no prior actions of divorce or annulment
between the parties except October 13th 2009 .
6. The Marriage is irretrievably broken.
7. Plaintiff has been advised that Counseling is available and
that plaintiff may have the right to request that the court
require the parties to participate in counseling.
8. Plaintiff requests the court to enter a Decree of Divorce.
I verify that the statements made in this Complaint
are true and correct. I understand that false statements made
herein are subject to penalties under 18 Pa.C.S. §4904, relating
to unsworn falsification to authorities.
