Notice is hereby given to Lisa Marie Depietro that Caroline Northup,
Petitioner, by and through her attorney, William M. Shreve, intends to take the
deposition of Dr. John Mira whose office is located at 3812 Market Street, Camp Hill,
PA 17011. The Deposition will be taken at Dr. Mira's office at 3812 Market
Street, Camp Hill, PA 17011, the office of Individual and Family Services. The
deposition will be take place at 1:00 p.m. on November 6, 2009. The deposition
is being taken to establish the incapacity of Lisa Marie Depietro.
