Decedent's wife on behalf of the Estate of Richard Foutz, deceased, as Petitioner, it is
hereby ORDERED AND DECREED that:
Life insurance proceeds issued by Beneficial Financial Group Insurance under
policy #BL0973551 shall be distributed to Mrs. Foutz and she shall retain said proceeds;
