PETITION FOR CHANGE OF NAME
The petitioner, TAMMY LYNN PATTON, on behalf of her minor daughter, CHELSEA
LYNN WOLFE, through their undersigned counsel, requests that this Court order a change of
name for CHELSEA LYNN WOLFE, pursuant to 54 Pa. Cons. Stat. Ann. § 701 et seq., and in
support thereof represent as follows:
Petitioner, TAMMY LYNN PATTON and her daughter, CHELSEA LYNN WOLFE
are residents of Cumberland County, Pennsylvania, reside and have resided at the following
location during the five years next preceding the filing of this petition: 602 Woodland Avenue,
Mount Holly Springs, Pennsylvania 17065, from April 22, 2000 to the present.
2. CHELSEA LYNN WOLFE was born on April 22, 2000.
3. Petitioner desires that her daughter, CHELSEA LYNN WOLFE, be known as
CHELSEA LYNN PATTON.
4. Petitioner was divorced from TIMOTHY DAVID WOLFE, CHELSEA LYNN
WOLFE's biological father, on February 8, 2007.
5. TIMOTHY DAVID WOLFE has granted Petitioner full and sole custody of
CHELSEA LYNN WOLFE.
6. Petitioner, formerly TAMMY LYNN WOLFE, filed a Notice of Resumption of
Surname with the Prothonotary of Cumberland County, Pennsylvania on November 4, 2009,
resuming the surname of TAMMY LYNN PATTON.

7. Petitioner desires that her daughter, CHELSEA LYNN WOLFE, be known as
CHELSEA LYNN PATTON to match Petitioner's surname, to preclude potential future
embarrassment and awkwardness for her daughter CHELSEA .
8. TIMOTHY DAVID WOLFE concurs with Petitioner's request that CHELSEA LYNN
WOLFE's name be changed to CHELSEA LYNN PATTON.
9. There are no judgments or decrees of like character of record against CHELSEA
LYNN WOLFE.
10. The Honorable Judge Ebert issued Petitioner's divorce decree. No other judge has
ruled on this or any other related issue.
11. Granting of petitioner's request would be in CHELSEA LYNN WOLFE's best
interest.
WHEREFORE, Petitioners requests that this Court enter a decree changing her daughter's
name from CHELSEA LYNN WOLFE to CHELSEA LYNN PATTON.

The petitioner is directed to give notice of the filing of the petition and of the date of the
hearing by publication in two newspapers of general circulation in this County, one of which
may be the official paper for the publication of legal notices in this county.
The petitioner is further directed to give notice of the filing of the petition and the date of
the hearing to any nonpetitioning parent of a child whose name will be affected by giving said
notice to the nonpetitioning parent.
