You are notified that a Mechanic's Lien Claim in the amount of $2,325.00 has been filed
on behalf of Shay's Home Improvements, LLC, against the property at 2A Round Ridge Road,
Upper Allen Township, Cumberland County, Pennsylvania, of which you are the owner or
reputed owner. The Claim was filed on /o , 2009, in the Court of

Claimant, Shay's Home Improvements, LLC, through the undersigned counsel, files this
claim against the improvements and property at 2A Round Ridge Road, Upper Allen Township,
Cumberland County, Pennsylvania, for the payment of a debt due to Claimant as contractor for
labor and materials furnished by Claimant in the alteration and repair of the improvements. In
support of the claim, the Claimant makes the following statement:
1. The Owner, or reputed owner, of the property is Linda Case, with an address of
2A Round Ridge Road, Mechanicsburg, Pennsylvania 17055.
2. The improvements on the property which are subject to this claim is a townhouse
with appurtenant land at 2A Round Ridge Road, Upper Allen Township, Cumberland County,
Pennsylvania, identified as Cumberland County Tax Parcel No. 42-29-2454-183 and more
particularly described in a Deed recorded in the Office of the Cumberland County Recorder of
Deeds on or about August 16, 2007, as Instrument No. 200732176.
3. The labor and materials for which the debt is due or furnished for the agreed sum
of $2,325.00 pursuant to written contract with the Owner dated August 31, 2009, a copy of
which is attached as Exhibit "A", which the Owner verbally consented to on or about August 31,
2009.
4. The labor and materials for which the debt is due consisted of repairs to the
property's poured concrete foundation and the charges for which claim is made is consistent with
the ordinary and customary charges for labor and materials for such a project.
The Claimant completed work at the property on August 31, 2009, which is less
than six (6) months before the filing of this claim.
6. Claimant has been paid nothing and there is due and owing a balance of $2,325.00
for which claim is made.
