Plaintiff acquired the obligation originally issued by Chase Bank/Heritage First USA. Plaintiff demands of Defendant(s)
the unpaid principal balance due under the credit card agreement Mastercard Acct. #5401683027090973. There is
due and owing from defendant the sum of $1.518.11. and reasonable attorney's fees, see attached statement and
Verification, attached (Exhibit "A").
In accordance with the Fair Debt Collection Practices Act, 15 U.S.C.A. 1692(g)(a) on May 29, 2009, Plaintiffs counsel
sent a written notice to Defendant. To date. Plaintiffs counsel has not received notification from Defendant that the
debt is disputed. (Sec: Exhibit "B").
